NDIS participants

Psychology that maps to your NDIS goals

Evidence-based therapy for adults, funded under Capacity Building — Improved Daily Living. Delivered by secure telehealth anywhere in Australia, or in person at your home or preferred location in specified suburbs located within Melbourne’s west.

Two ways to work together

Telehealth or in person

Same psychologist, same fee, same evidence-based approach. The right option depends on what suits your goals, your circumstances, and where you live.

Option 1

Telehealth — Australia-wide

Secure video sessions through Healthdirect Video Call — the platform trusted across Australian healthcare — or by phone where video isn’t suitable. No apps to install, no account to create.

How it works

1

Submit an expression of interest

Complete the form below. It captures what we need to check fit, funding and (for in-person) logistics — nothing more.

2

We review

Matt reviews clinical fit, your NDIS goals and capacity for the option you’ve chosen. If in-person isn’t possible, telehealth may be offered instead.

3

Service Agreement & consent

If we can proceed, you’ll receive an NDIS Service Agreement prepared from the details in your expression of interest. Review it, let us know if anything needs changing, then sign and return it online along with the consent form.

4

Confirm your start

Once returned, we’ll call or email to confirm your first session day and time.

Submitting an expression of interest doesn’t commit you to anything. Please note Salvere is not a crisis service — in an emergency call 000, or Lifeline on 13 11 14.

NDIS expression of interest

Takes about 10 minutes. Can be completed by the participant, a nominee or family member, a support coordinator or a plan manager. Press Send and we’ll email you an acknowledgement straight away.

1. Who is completing this form

So we know who to reply to.

Preferred contact method
2. Participant & NDIS plan details

Details must match the NDIS plan exactly — this is what appears on invoices.

How is the plan managed? *
Unfortunately we can’t proceed with agency-managed plans. Salvere is not an NDIS-registered provider. If the plan may change to self- or plan-managed at the next review, you’re welcome to get in touch then.
Is funding available under Capacity Building — Improved Daily Living? *
3. Clinical context & goals

NDIS-funded therapy must map to the participant’s disability and the goals in their plan. Brief is fine — we’ll explore in detail together.

Anything we should know to keep everyone safe and supported? *

Salvere is a general psychology practice, not a crisis or acute service. Please tick any that apply so we can consider whether we’re the right fit.

4. Preferred option & logistics

Choose one. If in-person capacity isn’t available, we’ll let you know whether telehealth could work instead.

Preferred option *
Telehealth preferences
Preferred days (tick all that apply)
Preferred times
In-person details
Safety disclosures (tick all that apply — this is a standard work health & safety check, not a judgement)
Will a nominee, guardian or responsible adult be present? * (required where the participant is under 18 or has a legal guardian)
5. Acknowledgements

These mirror what’s in the Service Agreement, so there are no surprises later.
